Accommodation Letter Request Form

Welcome to the Disability Services online Accomodation Letter Request Form. Three business days after you complete and submit this form, you can pick up your accomodation letters in the Disabilty Services office located on the first floor of the University Center (UC 101).

Here is an example of how to enter class information in the boxes below:
