Faculty Syllabus Statement and Referral


Faculty are strongly encouraged to place the following written statement inviting students to visit the Disability Services Office in their syllabus each semester.  This sample statement is the most recent suggestion from the office of civil rights on students disclosure.

Syllabus Statement: Students with disabilities who require accommodations (Academic adjustments, auxiliary aids or services) for this course must register with the Disability Services Office.  Please contact the Disability Service Office immediately in the University Center, Suite 101 or call 859-572-6373 for more information.  Verification of your disability is required in the Disability Services Office for you to receive reasonable academic accommodations.
 
In addition faculty are encouraged to verbally announce this statement to their class(es) at the beginning of each semester.

If the faculty member suspects that a student has a disability discuss the concern privately with the student and refer them to Disability Services.

If a student discloses a disability to the faculty member, the faculty member should request a verification letter from Disability Services before providing accommodations.

Guidelines for Test Proctoring  


Exam proctoring is a service provided by the office of Testing and Disability Services (TDS) at Northern Kentucky University to assist faculty with providing reasonable accommodations to students with documented disabilities.  Students eligible for exam proctoring will be given a letter from our office supporting the need for the specific accommodations.  The student should present this letter to the faculty member as soon as possible to begin the scheduling process.

Faculty members can provide the approved exam accommodations in their department if they so choose or send the exam (test, quiz, etc.) to Testing and Disability Services for proctoring.  Testing and Disability Services is available for the exam proctor accommodation, Monday – Friday 8:15 am – 4:15 pm.  Faculty should contact TDS if they have questions about this process.  Some extended hours may be available on Wednesday and Thursday.  Please contact TDS for further details.

Faculty Responsibilities  


  • Discuss with the student any exam accommodation concerns you both may have regarding the class.
  • Schedule with the student the time the exam is to be taken and where it will be administered.  Faculty can accommodate the exam in his/her own department or use the Disability Services exam proctoring resources.
  • If you use the exam proctoring service please deliver the exam to the office of Testing and Disability Services by the time you and the student agreed upon.  Hand delivering the exam is preferred because campus mail is delivered at various times to our department during the day and many tests are not received by the agreed upon time.  Exams should be delivered to University Center, room 101. E-mail delivery of the exam and tracking form is possible.  Please check the Disability Services Web page www.nku.edu/~disability, under Faculty Resources for directions on email exam delivery.
  • Provide the following essential information for each student using exam proctoring on the (lime green) tracking form:
  • Student's name
  • Course number and section
  • Faculty's name
  • Return information (building and room)
  • Allotted time given in the class for the exam (quiz, test, etc).  Please note that unlimited time is not provided.
  • Special instructions (open book/notes, calculator, corrections, etc.)
  • Please place a clear, legible copy of each exam in a separate envelope for each student.

 Student Responsibilities   


  • Obtain the verification letter for exam proctoring for each of his/her classes.
  • Present the verification letter to each faculty member in an office visit where student and faculty member can discuss the individual needs and concerns.
  • Schedule with the faculty member the time to take the accommodated exam and where it will be taken.
  • If the exam it is to be taken at TDS the student should sign up to take the exam at least one week prior to the date.  A calendar in UC 101 allows the student to sign up for an exam proctoring time as well as the reasonable accommodations for which the student has been approved. Scheduling of exams must include the time the exam is to be taken, the student’s name and class.
  • Remind the faculty member of the exam accommodation a class period before the exam.  (Pink reminder slips are available in the office of Testing and Disability Services)
  • Keep scheduled exam time, be prompt, and bring necessary exam taking supplies (i.e. pencils, blue book, calculator, etc.)
  • Maintain security of exam.  Discussion of the exam with other students before taking the exam or giving information to other students regarding the exam is cheating.  An exam should be taken as close as possible to the time and day that the class is taking the exam.

Testing and Disability Services Responsibilities  

  • Ensure the documentation and verification of disability for the student is correct.
  • Secure all exam material and keep it in a confidential and protected file.
  • Coordinate and supervise the administration of the exam proctoring service.
  • Maintain the integrity of the exam proctoring service and the exam while in the Testing and Disability Services office.
  • Return completed exam to the faculty member in a timely manner.
